چکیده

ADVANCES in heart surgery. catheterization, and angiography have called attention to constrictions of the pulmonary arterv and its branehes. Recenitlv SchumLacker and liurie' described a calcified stenotic zonie at the bifureation of the pulmlolnary arterv in a 14-vear-old girl. This was dilated with a Kelly clamiip. No postoperative physiologie studies were reported. Soiidergaard2 reported 3 cases with similar stenosis at the bifureation. Two of these were associated with the tetralogy of Fallot, while the third had an atrial septal defect ancd valvular pulmuonic stenosis. Although Sondergaard discovered the vascular obstructions at operation. n-tone was corrected. Several unoperated cases lhave been reported. Arvidsson ancd associates3 demonstrated by angiography what appeared to be multiple congenital sten-oses of the peripheral pulmonary arteries in 4 patients. Coles and Walker4 reported a case of bilateral constrietion of the maini branlches of the pulmuoniary artery, which was diagnosed bv catheterization and angiography. Eldridge, Selzer, and Hultgren'reported 5 patients Awith stenosis of a branlch of the pullmonaary artery demon strated by catheterization. We have recently encou-iitered 2 cases of stenoosis of the maini pulmonio+ary after-. Although Williams, Lange, and Hecht6 point out that sten-osis is probablv not unusual, our firsi

Approach for surgical correction of pulmonary artery coarctation without cardiopulmonary bypass.

Pulmonary artery coarctation often happens in patients with pulmonary atresia. The correction has been usually performed using cardiopulmonary bypass and during the neonatal period, influencing pulmonary artery development and prognosis. Three patients with pulmonary atresia with PDA underwent correction using median sternotomy without cardiopulmonary bypass. The PDA maintained the arterial sat...

Main indications and long-term outcomes of reoperation after initial repair of tetralogy of Fallot

Background and Objective: The aim of this study was to analyze our indications, surgical procedures, and clinical outcomes of patients undergoing reoperation after surgical correction of tetralogy of Fallot (TOF). Methods: Thirty seven consecutive patients who underwent reoperation late after intra-cardiac repair of TOF within a period of 10 years were assessed. Results: The most co...

Midterm results after surgical correction of total anomalous pulmonary venous connection

Abstract   Objective: To evaluate the surgical treatment of total anomalous pulmonary venous   connection (TAPVC) and determination of predictors for postoperative death.   Methods: Between 1995 and 2005, 80 patients aged from 1 month to 12 years underwent surgical repair for supracardiac (39), cardiac (34), infracardiac (3) or mixed (4) type of TAPVC. Systemic pulmonary hypertension (PH) in ...

Coarctation and balloons: inflated or realistic?

The use of catheters to dilate vascular obstructions began as early as 1964. In their classic article, Dotter and Judkins (1) described transluminal dilation of arteriosclerotic obstruction. It wasn’t until 10 years later that the use of balloontipped catheters in coronary artery dilations was reported by Gruentzig and Hopff (2) and later applied by Gruentzig and his colleagues (3). Since these...

Coarctation of aorta with right aortic arch. Report of surgical correction in 2 cases: one with associated anomalous origin of left circumflex coronary artery from the right pulmonary artery.

Two patients with right aortic arch, anomalous left subclavian artery, and coarctation of theaorta, have been operations for the relief of coarctation, successful in one but only partially successful in the other: in one case, a 'dacron' patch was inserted into the narrowed segment, but a mild recurrence of coarctation was found a year later; in the other, the proximal segment of the anomalous ...

Surgical Repair of Aortopulmonary Window in Infants

Background: Aortopulmonary window is a rare congenital heart defect resulting from a communication between the main pulmonary artery and the ascending aorta. Surgical closure is indicated in all patients with aortopulmonary window and should be performed at the time of diagnosis to prevent irreversible pulmonary vascular disease. Methods: From 1990 to 2002, 4 infants with aortopulmonary window ...
